PRIVACY & CREDIT HISTORY
Where in this section the applicant is a company or partnership, then the word “applicant” also includes the Directors of the company or the proprietors of the partnership and where the proprietor is a company then its Directors.
The applicant authorises the Master Pool Builders Association Australia (MPBAA) to obtain a credit report from Credit reporting agencies and other credit providers, containing personal and/ or commercial information relating to the applicant for the purpose of assessing this application for commercial credit, membership criteria and collecting any overdue payments. The applicant authorises MPBAA to give personal information including information concerning this application and the applicant’s credit worthiness, as allowed by the Privacy Act 1988 (Cth) to credit reporting agencies to obtain credit reports and allow the credit reporting agency to create or maintain a credit information file containing information about the applicant.
In accordance with the Privacy Act, the applicant authorises MPBAA to give and obtain from credit providers named in this application and credit providers that may be named in a credit report issued by a credit reporting agency information about the applicant’s credit arrangements. The applicant understands this information can include any information about the applicant’s credit worthiness, credit standing, credit history or credit capacity that the credit providers are allowed to give or receive from each other under the Privacy Act. The applicant also acknowledges that this will enable MPBAA to determine the applicant’s suitability for membership of MPBAA. MPBAA uses and discloses personal information collected in this application to open and operate the applicant’s account, to exchange information as described above and to market its products and services.
MPBAA may disclose this information to credit reporting agencies and other credit providers and as otherwise permitted under the Privacy Act. You may access personal information we hold about you subject to the Privacy Act, by contacting us at the office. The applicant agrees that, unless they request otherwise, information collected from the applicant may be used by MPBAA for marketing purposes. If this authority is not provided, the applicant understands that MPBAA may not be able to establish or operate an account for the applicant.
APPLICANTS DECLARATION
I/We have read and understood the enclosed Association rules, as set out in the Policy & Procedures Manual and Code of Ethics. And if I/we are accepted as a member of MPBAA, I/we hereby agree to be bound by the provisions of the Code of Ethics, Policy & Procedures, Constitution and any other rules as they may be amended from time to time, to support the aims and objects of the Association and to pay such annual subscription fees as the Board of Directors shall from time to time determine.
I / We hereby authorise MPBAA to make all enquiries which it may consider necessary or appropriate in the course of considering this Membership Application, such enquiries including but not limited to trade and customer references, whether listed in this Application or not, enquiries of the Office of Fair Trading, and from any other government body or authority as MPBAA may think fit. I/We acknowledge that MPBAA shall not be under any obligation to give any reason for rejecting this Application.
I/We understand that the submission of a payment does not mean acceptance or any other entitlement of membership of MPBAA and until a decision confirming membership is relayed to us in writing I/we are not entitled to use the Association emblem or in any way to indicate that I/we am/are a member of the Association.
I/We declare that all of the information contained herein is true and correct and accept all conditions of the application.
I/We will comply with any direction of MPBAA to undertake Industry Training as a condition of the processing of this application, or if the application is successful, in the
elevation of membership from Provisional to Full.
If the applicant is a company, by signing this form you agree that the company is solvent and able to pay all of its debts as and when they fall due for payment.
I/We declare that we will adhere to all standing national and State-based codes, regulations, and laws applicable to our field of commercial activity. Specifically, we will investigate and ensure Energy Safe Victoria, OHS, Building Code of Australia and Standards Australia regulations are maintained.
